Form fields were removed after a properties update

We’ve identified an issue that was causing certain form fields to be removed from forms. This issue started occurring at Jul 5th 12pm EST until Jul 11th 1pm EST. The root cause of this issue resulted in updates to a property causing them to be removed from all forms.
We have addressed the root cause of this issue and have been able to add these properties back into the majority of forms on July 14th 10:30am EST. Submissions during this period will not have contained the missing field.

A number of forms were unable to be reset back to their previous state. We will be contacting customers still affected by this issue to work towards a resolution on this.
